Yep, that's correct. Looked like the tab had bent just enough to let the bolt go. We'll see..
 
I thought the tab washer was perhaps a wee bit flimsy and I have a few in stock and replace every time I have that bit apart and a wee bit of Loctite on the bolt threads may also help

Thanks Brian, I considered loctite but because that bolt is long and thin. I was afraid it may break when trying removing it next time.
