Second gear

If you are talking about from first to second you are not completing the shift, you are hitting neutral. shift up a little more forcefully.
 
As Steve said you are most likely going into neutral as during that gear change you go out of first then into second and has perhaps a longer throw than other gear changes and needs a deliberate change rather than a flick of the toe
On my bike there is usually a clunk when second is fully selected

OLD SCHOOL:

Hold you shift into second Tightly UP if using your toe or DOWN using the Heal until it is IN the gear and running down the road... :s

Sometimes I DO feel the gear Drawing into that gear Tighter and completing it into the next gear it stays...

I do this on ALL my shifts UP or Down... Get into that Habit...:D It Works.....
